Jack was preceded in death by his parents. His survivors include his sons, Jerid (Ivy) Baker, Tyler Baker and Shelbi Hajek; daughters, Jennifer Baker and Shandi Hajek; 7 grandchildren and his sister Stacy Baker.
Cremation has taken place.
