Theresa E. Forman of Pueblo, Colorado, formerly of Johnson County, Arkansas was born on July 20, 1919 in St. Louis, Missouri to Louis and Fanny Viszy Nagy. She passed away on November 17, 2019 in Pueblo, Colorado at the age of 100. She was the Presiden of EH, a member of the Republican Women, member and organist at Hagarville Baptist Church, and a Cub Scout den mother. She is preceded in death by her parents; her husband, Curtis C. Forman; her brothers, Louis Nagy, Julius Nagy, and Jimmy Nagy; her sisters, Fanny Bognar and Mary June Lewis. She is survived by her sons, Douglas C. Forman of Branson, Missouris and David C. Forman of Lafayette, Colorado; her daughter, Debra C. Johns of Colorado Springs, Colorado; four grandchildren and nine great-grandchildren. Visitation is from 2 p.m. until 4 p.m., Sunday, November 24, 2019 at the Hardwicke Chapel in Clarksville, Arkansas. Funeral Services are 2 p.m., Monday, November 25, 2019 at the Hagarville Baptist Church in Hagarville, Arkansas with Reverend Kenny Preston officiating. Burial will follow at Hagarville Cemetery in Hagarville, Arkansas. Pallbearers are Dave Forman, DJ Forman, Anthony Evans, John Johns, Jerry England, and Bruce Garrett.
